What This Means on This Site
ClaimMeds connects individuals who submit the evaluation form with participating pharmacies and marketing partners. Some state laws define "sale" or "sharing" broadly enough to include this kind of referral. If you opt out below, we will not share your personal information with our pharmacy and marketing partners for that purpose going forward, and we will not use your session and analytics data for cross-context behavioral advertising. Opting out will not affect any request already in progress with a pharmacy or partner you have already been connected with.

You do not have to use this form. You may also submit a request by emailing contact@claimmeds.com directly. We will respond within the time required by applicable law.
Verification
To protect your information, we may need to verify your identity before completing certain requests (such as access or deletion) by asking you to confirm details you previously provided.
Authorized Agents
You may designate an authorized agent to submit a request on your behalf. We may require proof of the agent's authorization and may still require you to verify your own identity directly with us.
Non-Discrimination
We will not deny you services, charge you a different price, or provide a different level of service because you exercised your privacy rights.
Global Privacy Control
Some browsers and extensions send an automated opt-out preference signal known as Global Privacy Control (GPC). Where required by law, we honor valid GPC signals as a request to opt out of sale/sharing for that browser.
